The Memory Clinic Program offered at the Powassan & Area Family Health Team works with local community services to provide patients with care coordination, caregiver support and most importantly patient centered care. This program will allow patients to remain as independent and healthy as possible, with emphasis on providing ongoing support for the caregivers.
The Program consists of a multidisciplinary team, which receives referrals from our family health team physicians and nurse practitioners. Our team consists of 2 physicians, 2 clinical assistants, 1 social worker and 1 Alzheimer’s Society representative who communicate directly with the patient’s primary health care provider. We provide a comprehensive plan of care and education through the journey of memory loss.
Our goal is to assist patients to stay at home as long as they can in a safe environment with the assistance of their family/friends, our team and community partners. We assist patients and their caregivers so that we can prevent crisis situations at home as well as admissions to hospital. We have worked with our community partner The Alzheimer’s Society to provide education for patients and caregivers on living life with dementia.
