The Powassan & Area Family Health Team is proud to serve the patients in our catchment area. We are an interdisciplinary team of health care providers including a Nurse Practitioner, 2 Registered Practical Nurses, a Social Worker, a Psychotherpist and a part time Dietititian in addition to our seven family physicians. All of our providers work together to offer a truly collaborative, team approach to primary care.
The Powassan & Area Family Health Team provides a wide range of primary care services and also develops and implements evidence-based, collaborative chronic disease management programs, health promotion and a thorough preventative care program. Our team utilizes an EMR (electronic medical record system) and is always striving to incorporate more efficiency into our practice through IT solutions and integration. Our team is committed to quality improvement.
